Freins et leviers dans la pratique de la réduction des risques et des dommages en médecine générale lors de la prise en charge des patients consommateurs d’opioïdes en Basse-Normandie

Abstract : The general practitioner (GP) is an actor of primary care and in addictology particularly. We wanted to identify what were the different brakes and levers for the implementation of Harm reduction (HR) among opioid users in primary care, through the representations and practices of GPs. METHOD: We made a qualitative study using 13 semi-directiv interviews with GPs based in Basse Normandie. A thematic analysis was made by constituting a thematic tree using the Xmind® software. RESULTS: The obstacles linked to their practice were: lack of training in HR tools, legislation, isolation and lack of time. The obstacles linked to their representations were: their moral values and the representation of opioid users. The levers linked to the practice were: multi-professionalism, the GPs’s place in the healthcare system, training in HR tools and the promotion of complex acts. The levers linked to their representations were: the positive image of the definition and tools of HR, their positioning as an actor of HR. CONCLUSION: Through his unconditional welcome to patients, his global vision of the individual and his mission of disseminating the opioid substitution drugs entrusted to him, the GP is a central actor in HR. However, its lack of coordination with specialized structures, its lack of training and its place often limited to the health role of prescriber seems to limit their HR practices.
