Audit sur la prise en charge de la douleur postopératoire à l’Institut Gustave Roussy en 2018

Abstract : Introduction: the management of postoperative pain is subject of regularly updated recommendations. In 2011, the Department of Anesthesiology of Gustave Roussy Institute conducted a first audit to evaluate perioperative analgesia protocols. Seven years later, it seemed necessary to update them. The objective was to evaluate the effectiveness and application of these protocols and to compare them with the previous audit.
Patients and methods: all patients scheduled in the operating room over a 10-day period in March 2018 were eligible. Perioperative prescriptions, pain scores, morphine consumption between SSPI and day-4 were collected using the ALGOSFAR tool. 2018 data were compared to 2011.
Results: 274 patients were included. The use of NSAIDs improved compared to 2011, although efforts must be continued, especially in ENT surgery. The more systematic use of local anesthetics was associated with the most significant progress. The introduction of the paravertebral block in breast surgery particularly allowed a marked improvement (15% of ENS > 3 at day-0 compared to 90% in 2011). Areas for improvement have been identified: compliance with antalgic prescriptions and a more systematic use of opioids during hospitalization and the development of ambulatory surgery for minimally invasive and painless surgeries.
Conclusion: this audit evaluated the efficacy of our perioperative analgesia protocols and proposed corrective measures. This strategy of repeated audits seems an interesting approach to professional practices improvement.
