Performance diagnostique de la protéine c-réactive pour les infections intra-utérines et néonatales précoces dans le cadre des ruptures prématurées des membranes à terme : étude prospective portant sur 297 patientes pendant 1 an

Abstract : Objective: to study the predictive value of the C-reactive protein in cases of premature rupture of the membranes at term for probable early neonatal infections and intrauterine infections. Population and method: prospective monocentric study conducted at the maternity ward of the University Hospital of Bordeaux, from January 2020 to January 2021, involving 297 patients with a rupture of the membranes after 37 weeks. A CRP assay was performed for all patients as soon as they were admitted to the emergency room and diagnosed. The data was collected on DxCare, the reference computer software in our center. Results: out of the 297 patients, 50.5% (150/297) had a positive CRP, 48.4% (144/297) had a negative CRP and 1% (3/297) had an unknown value. One percent of the newborns had early-onset neonatal infection and no intra-uterine infections were reported. For a 5 mg/L threshold, the diagnostic performance of the CRP was: VPP 0.7% [0.2-3.8], VPN 99.3% [96.3-100.0], Se 50% [1.2-98.7] and Spe 51% [45.1- 56.9]. Of the 26 patients with initial CRP 15mg/L, none had a maternal-fetal infectious complication. Conclusion: only the negative predictive value of CRP appears to be relevant in the management strategy of premature rupture of membranes at term.
