Facteurs prédictifs de grossesse après insémination intra utérine : une analyse rétrospective de 584 cycles

Abstract : Goals: Identify the factors of success and failure of pregnancy after intrauterine insemination with partner sperm. This is a unicentric retrospective study of 265 couples for 584 cycles concerning all cycles of intrauterine insemination at the CHU of Caen from January 2014 to December 2017. Results: The pregnancy rate per cycle was 16.1%, live births rate per cycle 12.2%. The cumulative pregnancy rate was 35.7% and the cumulative live birth rate was 26.6%. Patients belonging to stage 1 of the Poseidon classification (aged less than 35 years, AMH> or = 1.2ng / mL, CFA> or = 5) and with idiopathic infertility had a significantly increased rate of live births compared to others stages (stage 1: 16.26%; stage 2: 6.73%; stage 3: 0%; stage 4: 7.69%; p: 0; 006). A number of progressive motile spz inseminated greater than 10M was associated with an increase in the rate of live births if the woman's age is less than 35 years (<5M: 9.76%; 5-10M: 9.72 %; > 10M: 18.42%; p: 0.03) Patients with unilateral tubal obstruction had a lower pregnancy rate than those without tubal pathology. (6.8% and 17.1%; p: 0.04). Conclusion: IUI provides a good solution in the treatment of infertility in the following cases: young patients with good ovarian reserve and idiopathic infertility with an inseminated spz count of more than 10M. Patients with unilateral tubal pathology should be referred for IVF from start.
