Étude sur les allergies médicamenteuses déclarées par le patient hospitalisé en service de Médecine

Abstract : Self-reported drug allergies by hospitalized patients are frequent and have consequences in terms of hospital morbidity and mortality. Most of them are mislabeled and are often non-allergic hypersensitivity reactions (or intolerance). In this prospective study, wedetermined the characteristics of self-reported drug allergies in an internal medicine unit from November 2020 to May 2021. 5.6% hospitalized patients reported drug allergies. Most of them concerned anti-infective drugs (58.7%). For 29.3% of self-reported drug allergies, physicians reported an impact on the health care management. Among those cases 70.8% led to a use of a therapeutic alternative. Only few reported drug allergiesseem to be real, becausemost symptoms described in the initial reaction were mild and resolved spontaneously. Six reintroductions could be performed without allergic reaction. We showed no significant difference between the length of stay, mortality and the number of intensive care transfer. There are significantly (p = 0.02867) more women among the patients reporting allergies. This may be explained by the predisposition to inducible urticaria in the female population, as most patients described cutaneous symptoms in the initial reaction. Despite the absence of hospital morbidity associated with a drug allergy reported by the patient in our study, the implementation of simple tools could quickly eliminate a drug allergy. It could reduce drug allergy impact on treatment, by limiting prescription of a drug alternative potentially providing adverse drug reactions, selection of resistant germs or more expensive cost.
