L'artérite à cellules géantes racontée par les patients : analyse du vécu de la maladie, de son traitement et de son impact sur la qualité de vie

Abstract : The objective of our study was to assess the quality of life (QOL) of corticosteroids (CS) treated patients with Giant Cell Arteritis (GCA). Ninety patients with GCA from Caen teaching Hospital (France) were included and compared with the practitioner recorded data from their own follow-up. QOL was measured using a specific questionnaire and standardized scales (SF-12 and GDS30). QOL was impaired, generating moderate to severe SF-12 disability scores in 46% of patients in the physical domain and 33% in the mental domain. Adverse reactions attributed to CS were reported in 81% of patients. Predictors for altering SF12 physical domain were age > 80 years (p=0.008), GCA lasting > 2 years (p=0.02), onset of diabetes mellitus (p=0.008), and reduction in muscle mass (p=0.008). Predictors of mental domain alteration included use of immunosuppressants (p=0.02), recording of an adverse reaction reported to CS (p=0.001), specifically depressed mood (p= <0.0001), and any infection requiring treatment (p=0.03). The General Practitioner plays a key role in coordinating patient monitoring and screening for risk factors impacting the QOL of patients with GCA. Monitoring including special attention to cardiovascular factors, walking difficulties and neuropsychological disorders could be optimized through the development of a specific rating scale for QOL in patients with GCA.
