Expositions professionnelles à la silice cristalline des patients souffrant de vascularite à ANCA au CHU de Bordeaux

Abstract : Objectives: our goal is to describe occupational exposures to silica of patients suffering from ANCA vasculitis at Bordeaux universary hospital (France), and to compare the professional and clinical impacts between the exposed and non-exposed groups. Methods: we have carried out a professional interview with patients suffering from ANCA vasculitis treated at Bordeaux universary hospital (France) according to the ARC and CHCC criterias. Occupational exposures have then been characterized by a crystalline silica expert. Results: the study was carried out on 82 patients suffering from ANCA vasculitis. 33% of patients were exposed to silica. Among them, 40.8% had a strong exposure, 29.6% had a medium exposure, and 29.6% had a weak exposure. The average duration of exposure to silica was 28.3 years, and the average latency between the start of the exposure and the start of the disease was 34 years.The exposed group mainly suffered from MPA (40.8%) (p = 0.05) with a more frequent lung and kidney involvement (p = 0.03). The exposed group were clinically worst, with a FFS score more frequently bad (p = 0.04) and a higher end-stage kidney failure frequency (p = 0.01). We did not find any significant difference concerning the professional impact of the disease between the exposed and the non-exposed groups. Conclusion: there is a high prevalence of exposure to silica in patients suffering from ANCA vasculitis in our study, and the exposed patients would be more severely ill than the non-exposed patients. Further studies of the same kind are needed to get a better knowledge of occupational exposures so that they can have a better support and recognition of their disease.
