Évaluation rétrospective des anti-TNFα dans la folliculite décalvante de Quinquaud

Abstract : Introduction: folliculitis decalvans (FD) is a therapeutic challenge for dermatologists. Objective: to evaluate the clinical efficacy of anti-TNF alpha (anti-TNFa) for the management of FD. Methods: a single-center retrospective study was conducted between January 2010 and December 2020 in the Department of Dermatology in Bordeaux and included patients with refractory FD treated by TNF alpha inhibitors. Patients were considered as controlled when they obtained an IGA score ≤ 2/4 at M12. Results: eleven patients were included, with a history of FD for an average of 8.55 ± 1.26 years prior to the introduction of infliximab (n=9) or adalimumab (n=2). Nine patients had failed at least two lines of systemic therapies before starting anti-TNFa. The median IGA score at baseline was 3. Five patients out of 10 were considered as controlled at M12. Overall, the safety profile of anti-TNFa was good. Discussion: we report the largest series of patients with FD treated with anti-TNFa. Furthermore, this is the first study to propound a severity score specifically designed to evaluate therapeutic interventions in FD. TNF alpha inhibitors allowed improvement of the disease in a group of patients with refractory FD. Our results suggest that the benefit of anti-TNFa should be observed after at least 6 months of treatment. However, further studies are required to assess the efficacy and the safety of this therapy for FD.
