Efficacité de l’azacitidine dans la prise en charge des complications auto-immunes et inflammatoires des syndromes myélodysplasiques

Abstract : Myelodysplastic syndromes (MDS) and chronic myelomonocytic leukemias (CMML) are often associated to auto-immune or inflammatory manifestations (AIMs). Efficacy of azacytidine (AZA) in the management of these AIMs is debated. We conducted a case-control study towards MDS/CMML treated by AZA in Nice University Hospital between 2005 and 2019 to compare epidemiologic and clinical features, survival, between patients with or without associated AIMs, and to evaluate efficacy of AZA in AIMs management.
20 patients had AIMs, 110 were used as controls (AIMs prevalence = 15%). WHO classification of MDS/CMML at diagnosis and IPSS at AZA introduction were statistically different between two groups (both p<0,01). Before AZA onset, 90% of AIMs had received a specific treatment (with steroids in 75%). 30% needed a second line regimen, 10% a third, and 5% a fourth line regimen. At AZA onset, 31,5% of AIMs patients were steroid-dependent. In those who didn’t achieved a complete response, 81% showed an improvement under AZA. Among all AIMs patients, 47% showed an improvement in AIMs response with a decrease in steroid-dependence rate to 5,5%.
No difference in overall survival was observed between the two groups (median OS 20,6 months). AZA appears to provide a benefit in terms of management and steroid dependence in AIMs patients with predominantly high-risk MDS/CMML.
