Valeur prédictive des performances perceptives précoces des patients adultes implantés cochléaires

Abstract : Aim : While many researches have recently focused on immediate activation after cochlear implantation, a few studies only have explored the issue of CI adults users’ preliminary assessment of speech recognition. The very first post-surgery CI users’ appointments are meant to activate the electrodes and adjust stimulation threshold fittings. The onset of speech recognition evaluation only starts from 3-month post-implantation, though short perceptive tests are already run from 1-month post-activation. The aim of our study is to explore a possible predictive aspect of the «early» scores when compared to long-term performances.
Materials/Methods : Data of twenty first-time CI adult patients with post-lingual deafness assessed with closed-set word test (days of the week randomly listed) at 7-day and 1- month post-CI activation and with monosyllabic word tests (Lafon lists) at 7-day, 1-month, 3-month and 6-month after implantation were analyzed.
Results: A predictive value in monosyllabic word recognition has been found between 1-month and 6-month, with a plateau effect at 3-month. The results of closed-set word test 7-day post-activation were also linked to monosyllabic word test outcomes at 6-month. However, performances in pre and post-implantation were not correlated as predictive of post-implantation outcomes.
Conclusion : Early speech recognition results at 1-month after implantation could be a relevant prognosis of long-term performances’ evolution profiles in order to adapt and maximize post-implantation rehabilitation. These predictive indicators could even be ascertained as early as days after activation, should immediate stimulation expand as a standardized procedure.
