Évaluation de la prévalence de la stéatose hépatique et de la stéatohépatite non alcoolique chez les patients asymptomatiques dysmétaboliques hospitalisés dans le service de diabétologie CHU Saint Pierre de novembre 2018 à mai 2019

Abstract : Background : Today, NAFLD is a major public health problem into the world in the context of increased metabolic disease prevalence above all in Reunion Island. It is the hepatic expression of these diseases. The aim of this work was to determin the prevalence of NAFLD and its stages in patients with metabolics diseases in Run Island and to observe caracteristics of patients suspects of NAFLD et advanced fibrosis. Method : A retrospective analysis of patients with metabolic diseases hospitalized in CHU of Saint Pierre was realized. They benefited from clinico-biologic assessment, abdominal ultrasound and Fibroscan to divide them between groups NAFLD, without NAFLD, NASH with advanced fibrosis (NASH+F3) and NAFL without advanded fibrosis (NAFL/SS). Advanced fibrosis was defined with clinico-biologics scores or Fibroscan. Results : 145 patients were included. NAFLD’s prevalence in obese population from Run Island was to 83,9% and this of NASH with advanced fibrosis to 42,8% or 16% according to the definition. In diabetic patients, 78% had NAFLD and 43% or 13% advancd fibrosis. In population with metabolic syndrome, 85,5% had a NAFLD and 52% or 16,7% advanced fibrosis. All of these prevalences were higher than national prevalence for NAFLD and advanced fibrosis whatever the definition. Morbid obesity, fat mass, waist circumference, metabolic syndrome and sedentarity were significatively most important in NAFLD group. Conclusion : In context of metabolics diseases very present in Run Island, NAFLD doesn’t constitute an exception with a prevalence significatively higher . A systematic screening is essential in care pathway of these patients.
