Évaluation de la prise en charge ortho-gériatrique de la fracture de l’extrémité supérieure du fémur au CHU de Caen

Abstract : Introduction: Hip fracture is a common serious injury afflicting the geriatric population. Many orthogeriatric strategies have been developed to improve effectiveness and security of care in elderly people. The aim of this study is to assess the care given in an orthogeriatric unit in hip fracture. Materials and methods: We conducted a single centre retrospective study at the Caen University Hospital (France). We compared 252 patients who received traditional orthopaedic care with 194 patients treated in an orthogeriatric unit including 2 geriatricians. Results: There was no significant difference between the 2 cohorts in 6 month mortality, duration of stay, institutionalization rate and walking abilities. In the orthogeriaric cohort, there was a significant reduction of time to surgery, readmission between 1 and 3 month and a better economic valorization of the hospital stay. There was more complication during hospitalization within the orthogeriatric care unit. Conclusion: Hip fracture is still very challenging. We didn’t observe beneficial effects of orthogeriatric care unit on mortality but some bias could have disturbed the results of this study.
