La première consultation gynécologique : témoignages croisés entre deux générations de femmes

Abstract : Objectives: the first gynecological consultation plays an important role in women’s life. It partly conditions the adherence to gynecological follow-up throughout life. Our study proposes to explore several elements of this first consultation (choice of professional, course and influence of intergenerational transmission), studying the testimonies of women who had their first consultation 30 years ago and those of their daughter, who had their first consultation in the years 2015-2016. Our objective was to describe the first gynecological consultation at two different times in order to study the evolution of practices and the importance of mother-daughter transmission in such a delicate field. Materials and method: our study consists of 26 semi-structured interviews for 13 mother- daughter couples conducted between October 2020 and December 2020. Mother and daughter were interviewed separately with the same questions, allowing us to compare the elements related to their first gynecological consultation at two different times. Results: according to the testimonies collected, the gynecological consultation is still mostly built on the "medical questioning then clinical examination" pattern. Only four women in the "girl" group were not examined. This consultation occured almost exclusively with a gynecologist, whatever the generation considered. However, one difference was highlighted: prevention has become an important and integral part of a medical consultation. The most striking change between these two generations is the change in intergenerational transmission. For the most part, Women aged from 20 to 26 years were, able to talk to their mothers about the first consultation and gynecology in general. In contrast, women of the previous generation had no such dialogue with their mothers. Conclusion: it was recognized through the different reasons for consultation that the place of gynecology has become more important in the lives of women. The intimate aspect of the examination is less apprehended by the younger generation, partly due to intergenerational transmission. The mother has a role to play in her daughter's first gynecological consultation. The importance of dialogue was emphasized by both mothers (who had not experienced this mother- daughter transmission) and daughters, who felt that their mother had played a positive role in this occasion.
