Revue Systématisée de la littérature : effet de la mindfulness sur le craving dans le domaine des addictions avec et sans substance

Abstract : Abstract: the objective of this thesis is to review, through a systematic review of the literature, the modulating action of mindfulness on craving in the field of addictions with or without substance. Methods: for the review of the literature, we first selected a list of articles extracted from PubMed and ScienceDirect. We have screened them using pre-established inclusion criteria. The publications selected were systematically analyzed according to the substance studied, the type of mindfulness training, methods of assessing craving, socio-demographic data, psychiatric and addict logical comorbidities and finally the effectiveness of mindfulness training in relation to crav-ing. Results: our review of the literature found 27 publications (25 RCTs and 2 case series), for a total of 3068 patients included. Among the addictions, 9 (33.3%) related to tobacco, 6 (22.2%) to poly-addiction, 5 (18.5%) to alcohol, 5 (18.5%) to food including 2 specifically on chocolate, 1 on the Internet Gaming Desorder and 1 on opi-oids. The mindfulness trainings were very heterogeneous between long trainings on the MBRP model or short ones. Different tools for evaluating craving have been found. Ten studies (37%) showed a significant decrease in craving, mainly in long mindfulness training. Conclusion: this review of the literature reveals encouraging results regarding a beneficial effect of mindfulness in the field of addictions, although the various works found are not homogeneous with respect to their mindfulness protocols or to the tools for evaluating craving. Several well-conducted and re-producible studies lead us to believe that this tool should be used in our care for patients suffering from a dual, psychiatric and addict logical pathology.
