Influence de la pratique intensive du violon sur la coiffe des rotateurs, évaluée en isocinétisme

Abstract : Background: upper string players have been reported to be among the most vulnerable to Playing Related Musculo-skeletal Disorders (PRMD) due to the high physical demand. However data on the underlying causes is scarce. Research are needed to explore the impact of violin practice on musculoskeletal system of musician. Objective: assess shoulder rotator cuff balance, strength, and endurance in professionals violinists, with comparison to a control group. Methods: the control group and the violinists group were homogeneous in terms of age, weight, sexe ratio, height and score Quick-DASH. Assessment was carried out with the isokinetic dynamometer Contrex®. The ER and IR muscles were concentrically assed at speeds of 60° and 180°/sec. The parameters chosen were: peak torque (PT) normalized by weight, and ER/IR ratios. Endurance was assed at 240°/s with the calcul of a fatigue index. Results: no significants differences between the dominant and non-dominant shoulders in IR and ER muscle concentric strength in violinists were noted and no significants differences between the violinists sample and the control group were noted. Conclusion: intensive practice of violin is not associated with muscle imbalance or rotator cuff weakness for healthy musicians. Further prospective studies are needed to compare the muscular balance and muscular endurance of musicians with pathology and those without. An appreciation of the role of these specific factors will undoubtedly be of great value in future prevention and treatment strategies for musicians with PRMD.
