Prise en charge des fractures péri-prothétiques de hanche au CHU de Caen de 2009 à 2019

Abstract : Periprosthetic fracture is one of the major complications of hip arthroplasty. The management is conditioned by the stability of the femoral stem. Thus, using the Vancouver classification and the therapeutic tree described by SOFCOT in 2005, we will move towards open reduction and internal fixation or implant(s) revision. The main objective of this retrospective and comparative study was to analyze the survival of the procedure according to the type of surgery. The secondary objectives were: the comparison of survival of the patients, the functional evaluation and the analysis of the failures. Have been compared 70 open reduction and internal fixation (O group) with 68 implant revisions (R group). Results: No significant difference for the survival of the procedure at the last follow-up (p= 0.12). No significant difference regarding the survival of the patients at 1 year (p= 0.59). However, the functional scores according to the Parker, PMA and HSS scores were significantly worse after surgery in each group (p< 0.01). Nevertheless, the comparison of scores after surgery were significantly higher in R group compared to the O group (PMA: p< 0.01 and HSS: p= 0.031). Lastly, bone consolidation was significantly better in the R group (p= 0.032). This work did not allow to find a significant difference in survival of the procedure or of the patient. In contrast, patients were feeling significantly better and had a superior bone consolidation in the R group compared to the O group. In cases where stem stability is uncertain, this work stands in favour of implant(s) revision surgery, compared to the open reduction and internal fixation.
