Comparaison d'une endoprothèse à libération de biolimus en Cobalt-Chrome à une endoprothèse en acier inoxydable portant le même polymère utilisée dans le traitement des syndromes coronariens aigus

Abstract : The objective was to see if the alliage of stent CoCr-BES Biomatrix Alpha, face to BES Biomatrix Flex and Neoflex in stain steel as an impact on the outcome of major adverse cardiac event at 12 mouths. It was an monocentric study on a prospective cohort of patients included from 2015 to 2018. Were eligible all patients aged 18 years or over admitted for acute coronary syndrome with or without ST elevation or unstable angina into the CHU de CAEN, and who had at least one lesion with stenosis of diameter of 50% or more. A Cox model was used to assess the relationship between the occurrence of MACE at one year and the type of stent implanted. A total of 412 patients were included. 157 (38.1%) in the CoCr-BES group versus 255 (61.9%) in the BES group. The average age was 65. The population consisted of 28.2% women, 238 (57.8%) patients were admitted for STEMI; 156 (37.8%) NSTEMI; 18 (4.4%) unstable angina. The initial socio-demographic characteristics were comparable. The duration of the procedure was on average 45 minutes in the 2 groups; On the other hand, the ray dose was significantly higher in the CoCr BES group (3505.6 cGy.cm2 versus 2978.7 cGy.cm2 p= 0.04), the contrast volume lower in the CoCr-BES group 156.6ml versus 168.6ml (p= 0.038). There was numerically less MACE at one year in the CoCr-BES 8 group (5.13%) versus 19 (7.5%) in the BES group but statistically not significant (p= 0.34). Our study confirms the safety and efficacy profile of CoCr-BES compared to BES but did not show a significant difference in MACE at 12 months.
