Intérêt du ré-entraînement spécifique des muscles respiratoires dans la maladie de Parkinson

Abstract : Background: The damage of lung function has been identified as a non-motor disorder in patients with Parkinson's disease. A decrease in the strength of the respiratory muscles and a feeling of dyspnoea may appear at a moderate stage of this disease, thus altering the patient’s quality of life. Objectives: To demonstrate the effectiveness or otherwise of Expiratory Muscle Strength Training or Inspiratory Muscle Strength Training in improving respiratory muscle strength, available respiratory volumes and decreasing dyspnoea. Method: Inclusion criteria were clearly defined: subjects had idiopathic Parkinson’s disease, with a moderate stage according to the Hoenh and Yahr classification. The following endpoints were analysed: peak respiratory pressures, forced vital capacity, the maximal expiratory volume in seconds and subjective perception of dyspnoea. The PubMed, Cochrane, PEDro databases and the resources of the University Library of Aix-Marseille. University were used to identify studies corresponding to our PICO criteria. A total of 4 studies were selected. Results and analysis: There was no significant change in forced vital capacity and maximal expiratory volume in seconds. Maximal expiratory pressure was increased after EMST training. Maximal inspiratory pressure is slightly increased after IMST training. Dyspnoea was reduced after IMST. Discussion: The effectiveness of EMST or IMST in this context cannot be stated given the methodological quality of the studies included in this review. However, the significant results obtained may lead to further studies with larger sample populations and a more rigorous protocol.
