Stigma of people with addiction by health professionals : current knowledge. A scoping review

Abstract : Background: stigma of people with addiction (PWA) is a long-known phenomenon, shared by general population, health professionals and patients themselves. Our aim was to assess the stigma of PWA by health professionals, its characteristics and its evolution. Methods: we chose to conduct a scoping review of the literature. A systematic search of PubMed, Scopus and PsycINFO databases targeted only literature reviews. Results:. from the 19 selected reviews, it appears that 20% to 60% health professionals had negative attitudes/beliefs about addiction. Addiction training and clinical experience with PWA were associated with a less negative attitude. Health professionals’ negative beliefs, lack of time or support were associated with less involvement in addiction care. A tobacco use disorder (TUD), illegal substance use disorder (ISUD), a relapse, a comorbidity or a criminal record were associated with a more negative attitude. The influence of several variables was inconsistent (personal experience, professional category, age, gender, education level). Evaluation of stigma changes over time seemed to show a decrease in health professionals’ attitudes between the 1960s and the 2000s. Conclusions: stigmatization of PWA by health professionals persists, and some variables that contribute to this trend could be changed: moral model of addiction, health professionals’ negative beliefs, lack of training, time, and role support. Teaching addiction specialty according to a medical model of chronic disease, and developing stigma-focused training, could enable caregivers to improve their attitude and remain faithful to the fundamental principles of medical deontology.
