Intérêt de l’utilisation du dossier pharmaceutique lors de la conciliation médicamenteuse en infectiologie et pneumologie dans le cadre de la situation sanitaire exceptionnelle liée à la pandémie de COVID-19

Abstract : During the COVID-19 pandemic, pharmaceutical interviews could not be conducted during the entry medication reconciliation. It was decided to measure the contribution of the pharmaceutical record (PD) as a source of information. 151 patients were included, divided according to the access to the PD and the period of collection: either usual procedure allowing the ordinary analysis, or degraded period, related to the pandemic. The main objective was to compare theoptimized medication reconciliation (OMR) with the entry prescription of patients with a powered PD, compared to those without a PD, in order to verify the impact of the latter through different situations. We also noted the contribution of the PD in the searchfor history of antibiotic therapy, vaccination and self-medication, and measured the time taken to complete the OMR. When comparing the OMR to the entry prescription, no significant difference was found in the number of discrepancies detected, regardless ofthe group. On the other hand, more treatment changes were perceived in the PD group, with a higher number only because of the PD in a degraded period. It also helps to collect the history of antibiotic therapy and becomes essential in the search for the historyof vaccinations and self-medication products when the interview is not feasible; the latter being the most contributory source. It can be concluded that the PD consolidates the medication history among the usual sources and becomes essential to use in a degradedsituation, although it has limitations such as the absence of dosage.
