Stress parental lié à la fièvre : évaluation lors d’une consultation aux urgences pédiatriques de Bordeaux et impact sur le parcours de soins dans les sept jours suivant la sortie

Abstract : Objective: to evaluate the variation of stress level felt by parents presenting to Pediatric Emergency Department with fever and its influence on return visits in primary care and Emergency Departments within 7 days after discharge. Methods: We included in a monocentric prospective cohort families of children more than 3 months old, free of severe chronic disease, presenting with fever to Bordeaux Pediatric Emergency Department for 30 days between May and November 2018. Parents had to fill a self-administered survey including a visual analogue stress scale from 0 (no stress) to 10 cm (higher stress level imaginable). Evaluation of return visits within 7 days was made by phone call 8 days after discharge. Univariate analysis and multivariate regression logistic were performed. Results: Out of 177 families included, 158 answered the call. Median stress level after visit was 2 (Q1-Q3 = 0.7-4.5) concerning mothers and 1,3 (Q1-Q3 = 0.2-4.2) concerning fathers. Median stress level variation was 2.3 (Q1-Q3 = 0-4.3) concerning mothers and 1.1 (Q1-Q3 = -0.3-3.9) concerning fathers. 69 patients (43.7%) made a return visit, 10 of them (6.3%) in an Emergency Department. Stress level variations did not influence return visits, whether concerning mothers (p = 0.3953) or fathers (p = 0.9865). Fever persistence of 2 days or more (OR = 13.4 [5,7-31,4] ; p < 0,001), new symptoms (OR = 3.4 [1.3-9.0] ; p = 0.0124) and medical referral (OR = 2.9 [1.2-6.8] ; p = 0.0151) were significantly associated with return visits in multivariate analysis. Conclusion: Our study did not find results to support the hypothesis of parental stress level influence on return visits. However, this may be due to its low statistical power.
