Évaluation du parcours coordonné pour l'aide à l'arrêt du tabac en Gironde : étude rétrospective des prises en charge sur trois années

Abstract : Introduction: the fight against smoking has been at forefront of public health stakes in France. The general practitioner, at the heart of the overall care of the patient, has a privileged role to support him in his smoking cessation. Since 2017, the Bouscat Suburban Hospital offers a joint care between the addictologist and general practitionners treating patients requesting help to quit smoking. A first evaluation carried out in 2018 on a small sample (n=31) showed that smoking cessation was maintained for more than 6 months for 60% of the patients who started the pathway. The main objective of our tork was to assess the effectiveness of this approach, on a larger population. Methods: this is a quantitative retrospective study of the smoking cessation of patients included in the device between September 2017 and October 2020. The primary endpoint is the maintenance of smoking abstinence for at least 6 months. Results: among the patients who started the shared process (n=163/173), 55% maintained smoking abstinence for at least 6 months. Among the patients jointly treated before the health crisis linked to the COVID pandemic in 2020, the rate of maintaining smoking abstinence at 6 months is 62% (59% when counting those lost to follow-up as failures), which confirms the results of the originator study. The patients involved their attending physician in their follow-up by consulting him on average 2 to 3 times. Over 90% of practitioners wrote comments in the shared booklet. This support served as link between the two professionals and allowed the general practitioner to take ownership of the process. Conclusion: this city-hospital partnership model seems effective. Its diffusion in other regions, and the highlighting of the involvement of primary care physicians, could be interesting prospects for increasing the management of smoking cessation in the national territory.
