Radiomics et néphroblastome : apport dans le diagnostic différentiel et dans la prédiction de la réponse au traitement

Abstract : Purpose: to evaluate a CT-based radiomics analysis in children to differentiate Wilms tumor (WT) from other pediatric kidneys tumors before neo-adjuvant chemotherapy and to assess treatment response. Methods: this monocentric retrospective study consisted in analysis of segmented baseline and pre-operative CT scans of 37 children with WT (between 2010 and 2018) and 13 children with other renal tumors (between 2002 and 2018). medical records (including the histopathological to create models using machine learning. Non-parametric tests were performed in order to highlight significant associations between the distribution of features and the different classes (Wilcoxon-Mann-Whitney test and Fisher’s exact test). A Leave One Out Cross-Validation procedure was used to evaluate performance. Results: the predictive algorithm to distinguish WT from its differential diagnosis had a sensitivity of 0.897, a specificity of 0.615 and an AUC of 0.771. The most valuable criterion was based on heterogeneity. We were also able to predict partial responders to chemotherapy thanks to the baseline CT, using RECIST criteria: the algorithm showed a sensitivity of 0.789, a specificity of 0.824 and an AUC of 0.864. The main result was that partial responders enhanced more intensely than stable tumors. Conclusions: this preliminary study demonstrated the ability of radiomics to help radiologists for the differential diagnosis but could also help oncologists in order to personalize treatment. This study was performed on a small cohort, which could be confirmed by further multi-center studies.
