Identification des facteurs de risques transfusionnels chez les patients opérés d’une fracture de l’extrémité supérieure du fémur au CHU de Bordeaux afin d’établir un protocole d’épargne transfusionnelle

Abstract : Background: hip Fracture is a frequent pathology, the population at risk is increasing as well as its morbimortality. In this context, various perioperative therapeutic attitudes of blood saving ("patient Blood management") are possible with in particular the use of tranexamic acid, iron, the management of antiaggregants and anticoagulants and the respect of transfusion thresholds without established consensus. The main objective of the study was to evaluate the incidence of transfusion and to identify the factors associated with transfusion in patients operated on for hip fracture at the Bordeaux University Hospital in order to propose a "patient blood management" strategy to these patients. Method: we conducted a retrospective observational monocentric study conducted in the Bordeaux University Hospital for all patients operated on for hip fracture in 2019. 259 patients were eligible and had no exclusion criteria. Results: 42% of the men and 32% of the women were anemic according to the WHO definition, 134 red blood cells (RBC) were transfused in 70 patients, i.e. 27% of the total number. Multivariate analysis showed that age [OR 1.053 (1.012-1.097)], estimated blood loss [OR 1.005 (1.002-1.007)], hemoglobin at D0 [OR 0.325 (0.238-0.444)] and presence of hematologic diseases [OR 89.546 (1.000-8017.570)] were independent risk factors for transfusion. Conclusion: âge, significant blood loss, low D0 hemoglobin and a history of hematological disease are independent risk factors for transfusion.
