Relations des patients vasectomisés à leur médecin généraliste : expériences, attentes et interactions

Abstract : Background: Vasectomy is one of the most reliable methods of contraception. This method of contraceptive sterilisation is the male equivalent of tubal ligation, but it is considered "simpler, safer, easier and less costly" by the World Health Organisation (WHO). It is widely used in several countries, such as the UK, where it is performed on nearly 20% of men. However, vasectomy is not widely used in France: only about 1% of men have it and it is five times less common than tubal ligation. These differences can be explained by differences in information and representations related to this practice, both in the medical profession and in society. Objectives: To identify and understand the roles of the general practitioner in the contraceptive journey of men who have chosen vasectomy, based on the experience of these patients. Method: Qualitative study carried out by means of semi-structured interviews with 12 vasectomized participants. Dual recruitment modality via a urologist and via social networks. A thematic analysis with double coding was performed. Results: The general practitioner is generally not the initiator of the pathway leading to vasectomy. Very often, he accompanies a decision already taken by the patient who has sought information beforehand. The degree of involvement of the GP varies from an administrative role to a role of arbitration; there are gaps between his actual role and his expected role. The men interviewed had several expectations of their GPs, in particular that they would listen better, be made aware of the issue earlier and be given more precise information on vasectomy. Conclusion: The role of the GP in the pathways of vasectomised men is often a minor one which is not always in line with the patients' expectations.
