Évaluation de la pertinence diagnostique de la mesure isotopique du volume globulaire en cas de suspicion de polyglobulie : une étude de cohorte rétrospective

Abstract : Objective: Assess the performance and the d1agnosis relevance of cell volume isotopie measure, when polycythemia is suspected. Method: Single centre exhaustive recruitment of 156 patients who received, between January 2017 and December 2018, an isotopic measure of their cell volume by a marking of red blood cells with Chromium-51. Retrospective collection of haematologic data spanning at least two years post-test. Results : Out of the 156 patients analysed, 46 have a cell volume measure 125% superior to the theoretical norm (29,5%). Despite a poor agreement with the blood cell count data, it was not possible to highlight a situation in which the examination result could have been invalidated along monitoring. 13 patients out of 46 displaying absolute polycythemia had been filed later on as it being likely a secondary origin. The polycythemia vera origin had been retained for 4 patients out of the 46 ones, for whom 3 presented a mutation JAK2. ln case of wrong polycythemia, no significant difference had been highlighted between the haemoglobin values collected before the examination and those collected at least three months afterwards. Conclusion: The isotopic measure of cell volume is frequently made as a second-line auscultation. lt allows within this high-risk population for a plasma compartment tightening to materialise in an efficient manner an absolute polycythemia. lts result will enable to guide the patient care. Therefore this examination enables a correct diagnosis in most of uncertain cases, with good reliability.
