L’examen de l’épaule douloureuse chronique en téléconsultation : étude comparative interventionnelle de concordance sur 4 tests de référence de l’épaule douloureuse chronique en présentiel versus en téléconsultation

Abstract : Study: Interventional comparative study of concordance on four reference tests of the chronic painful shoulder, in face-to-face versus teleconsultation. Objective: To evaluate the concordance of four selected shoulder tests, carried out with the guidance of a general practitioner in teleconsultation, versus face-to-face by an orthopaedic surgeon. Context: Chronic shoulder pathologies are a frequent reason for consulting a general practitioner. The current SARS-CoV-2 coronavirus pandemic has been marked by a significant increase in teleconsultations, a development strongly encouraged by the public authorities. However, there is almost no analysis of the literature dealing with the evaluation of the semiology and validity of distance examinations. Methods and measures: Interventional, sequential study of the Test/Re-Test type, blinded, prospective, monocentric. For the statistical analysis, the Wilson score method will be used to calculate the proportion of concordants with the 95% confidence interval [CI]. The calculation of the required number of subjects will be based on the same Wilson score method. Expected results: Concordance of test results between the teleconsulting doctor and the standard gold represented by the specialist if the Kappa coefficient is higher than 0.80 for each test. Expected benefits: This research may contribute to the development of teleconsultation by broadening and adapting the semiological field to this new development. The use of the four tests in cases of chronic shoulder pain could be part of the guidelines for training general practitioners to use the teleconsultation tool, so that these consultations are efficient and therefore reassuring for both the patient and the practitioner.
