HSA-STRAIN : détection de la dysfonction myocardique au cours de l’Hémorragie Sous-Arachnoïdienne de bas grade clinique (WFNS 1-2) par échocardiographie de Speckle-Tracking (STRAIN)

Abstract : Background: stress cardiomyopathy is a common complication of subarachnoid hemorrhage (SAH) which worsens the neurological and vital prognosis. Strain imaging seems to have better sensitivity than the usual echocardiographic markers for diagnosis of left ventricular dysfunction. For the first time, we aimed to define the prevalence of stress cardiomyopathy during good-grade SAH (WFNS 1 or 2) using strain imaging. Materials and methods: 116 patients with good-grade SAH (WFNS 1 or 2) were prospectively included on admission to neurocritical care. Trans-thoracic echocardiography (TTE) was performed on day 1, 3 and 7 after bleeding. Routine measurements, including left ventricular ejection fraction (LVEF), were collected. Offline analysis was performed by a blinded examiner, to determine 2-, 3-, 4-cavities longitudinal strain and global longitudinal strain (GLS) of left ventricle. GLS was considered altered if it was ≥ -20%. Results: GLS was obtained in 33 patients, and 60.6% of them had impaired GLS on day 1, versus 1.6% with LVEF < 50%. Troponin elevation and ECG abnormalities on day 1 were respectively present in 50% and 52% of patients. No improvement in GLS was observed over time. However, we found a good correlation for repeated measurements between GLS and 4-cavity longitudinal strain (R= 0.74; p < 0.001). Conclusion: strain imaging reveals a higher prevalence (60.6%) of stress cardiomyopathy during acute phase of good- grade SAH (WFNS 1 or 2) than previously describe in the literature.
