Prise en charge diagnostique et thérapeutique de la grippe saisonnière aux urgences de l’Hôpital d’instruction des Armées Robert Picqué au cours de l’épidémie 2018-2019 : évaluation des pratiques professionnelles

Abstract : Background: seasonal influenza is a common illness that represent a major public health issue. It is responsible for an increase of winter mortality, especially for older and vulnerable people. Although its diagnosis is mainly clinical, new tools recently emerge including Rapid Diagnostic Orientation Tests (RDOT) now available. Therefore, we evaluated the professional practices using RDOT for seasonal influenza, as well as on its diagnostic and therapeutic patient management, in the Robert Picque Army Hospital’s emergencies. Method: we conducted an observational, descriptive, retrospective and mono-centric study on Robert Picque emergencies. The study was performed from October 18th 2018 to May 08th 2019. Firstly, we assessed the test relevance in terms of using indications and conditions. Secondly, we described the population admitted to HIARP emergencies benefited from a RDOT realization and the management of patient receiving a RDOT in our emergency department. Results: only 28.5% of the RODT were indicated. Test deployment dates were too broad. Many patients have been lately tested. Time spent by patients receiving RODT in HIARP emergencies was higher compared to the national average. 97% of patients received at least one supplementary examination, and 35.1% of patients received at least one treatment. Finally, 71% of patients receiving RDOT were subsequently hospitalized. Conclusion: efficiency of these tests utilization is only 28.5%. We could propose supplemental training for physicians on the RTOD optimal use and indications. The RDOT remains a fast, inexpensive and reliable diagnostic tool which need to be used wisely and integrated into a relevant diagnostic support. The 2020-2021 influenza epidemic did not occur, probably because of application of barriers measures. Therefore, it is questionable whether the strict application of the barrier measures during the influenza epidemic, would be more effective than a massive use of RTOD.
