La stimulation du nerf vague dans le traitement des épilepsies pharmaco-résistantes de l'enfant. Étude descriptive de 71 cas pédiatriques au CHU de Bordeaux

Abstract : Introduction: epilepsy is the most common neurological disease in children, in 30% of cases it is drug-resistant. Vagus nerve stimulation is an adjunctive treatment for drug-resistant epilepsy in France. The purpose of this study is to analyze the response to vagus nerve stimulation, in particular its efficacy in a pediatric cohort. Material and methods: we carried out a descriptive retrospective study of 71 pediatric cases implanted at the Bordeaux University Hospital between 2010 and 2020. We studied the response to stimulation with efficiency on seizure frequency, intensity, predictors of better response, and optimal stimulation parameters. Results: in this cohort, 59.7% of patients are responders and among them, 44% of patients with Lennox-Gastaut syndrome. There is a significant relationship between responder patients and type of epilepsy, in favor of generalized epilepsies. No other significant association was found between demographic and clinical characteristics of patients and significant efficiency in epilepsy. However, our results suggest that one of the predictors of better efficiency is early implantation of the vagus nerve stimulator, especially within 2 years of the onset of epilepsy. A dose-response effect of the stimulation is demonstrated. Conclusion: analysis of our patients' response to vagus nerve stimulation shows real benefits, whether on epilepsy, mood or behavior, with very few serious side effects. A better response would be favored by early implantation and a sufficient daily dose which remains to be determined.
