Suivi de la réplication du Torque Teno Virus dans le cadre de la prévention des complications infectieuses chez les patients greffés rénaux au CHU de Caen

Abstract : Torque Teno virus (TTV) is a recently discovered virus. The spectrum of illnesses associated with TTV is not yet identified. Studies suggest a link between plasma TTV viral load and host immune status. The aim of our retrospective study is to determine the kinetics of TTV replication in a cohort of patients who received a kidney transplant at CHU Caen over an inclusion period of 6 months, from January 01 to June 01, 2019, and to assess the relevance of monitoring this virus in the context of the prevention of infectious complications in kidney transplant ecipients. The follow-up protocol includes 7 pairs of samples (blood/urine) in the post-transplant year: D0, D7, D14, M1, M3, M6 and M12. For the 21 patients included, 143 plasma/urine pairs were analyzed. The mean plasma and urine viral loads were 2.80 +/- 2.50 log cp/mL and 1.38 +/- 1.67 log cp/mL, respectively. They were significantly correlated. Patients with at least one infectious complication had significantly higher TTV plasma viral loads. This correlation was stronger in the month following the transplant. In our study, a plasma viral load greater than 5.38 log cp/mL is associated with an increased risk of infectious complications. TTV appears to be a predictor of early infectious complications after kidney transplantation. A prospective study on a larger population would be necessary to confirm these results.
