Prise en charge des exacerbations aiguës de BPCO en médecine générale à Paris

Abstract : Introduction - The management of acute exacerbations of chronic obstructive pulmonary disease (AECOPD) is a public health issue in terms of morbi-mortality and economic costs. The main purpose of our study is to realize an updated observation of the management of COPD exacerbations by general practitioners (GP) in Paris and to look for a match with French guidelines (SPLF). Materials and methods - This prospective, descriptive and analytical investigation is based on datas collected with a computerized questionnaire. The target population is the Parisian general practitioners without side activity and who did accept to take part in the study. Results - Sixty-seven doctors took part in our study, 61 % were aware of the guidelines. An assessment of the transcutaneous oxygen saturation was systematic for 61 % of the respondents, 51 % prescribed an antibiotic therapy according to the SPLF, 67 % chose an appropriate antibiotic and 15 % advocated the recommended duration of 5 days. Half of the general practitioners (55%) prescribed systemic corticoids following the official guidelines. Conclusion - The COPD exacerbation management by GPs is mostly conformed with French recommendations however not all doctors use a pulse oximeter, and indications for antibiotic therapy are followed by only half of the respondents. In order to improve the practices and the health care system, promoting medical training seems essential. Continuous professional development training should be more in line with the needs of general practitioners. The study of the impact of the summary sheet created as part of our research work could be relevant.
