Caractérisation tissulaire myocardique dans la cardiomyopathie dilatée idiopathique avec insuffisance cardiaque aiguë en imagerie par résonance magnétique via les séquences mapping

Abstract : To discriminate idiopathic dilated cardiomyopathy with acute heart failure (IDCM-HF) from stable patients (IDCM-noHF) comparing structural and functional cardiac magnetic resonance imaging (cMRI) left ventricular (LV) measures with quantitative T1 and T2 mapping parameters. LV volumes, ejection fraction (LVEF), myocardial native T1, post-contrast T1, T2, extra-cellular volume (ECV), extra-cellular and intracellular mass index were measured in 44 individuals (average age: 46 years, 28 men): 22 IDCM patients without coronary artery disease (11 IDCM-noHF and 11 IDCM-HF with new onset acute heart failure) and twenty-two age and sex-matched healthy controls. LV parameters were compared between controls and IDCM with and without HF. Native T1 and extracellular mass were increased in IDCM patients with acute heart failure compared to stable IDCM patients with similar disease severity. cMRI provides sensitive biomarkers of myocardial tissue properties that may be dependent on the hemodynamic status potentially relevant to their diagnostic of prognostic significance in IDCM, (native T1: 1066 ms (1054-1092) vs. 1042 ms (1012-1060) p = 0,013 and extracellular volume significantly higher 33 (31-36) vs. 29 (26-31) p = 0,014. Indeed, ECV was the overall best parameter to identify the presence of HF in IDCM patients with a cut-off value of 30 % (AUC=0.93, Se = 91%, Sp = 85%, accuracy = 86 %). cMRI provides sensitive biomarkers of myocardial tissue properties that may be dependent on the hemodynamic status potentially relevant to their diagnostic of prognostic significance in IDCM.
