L’Accouchement accompagné à domicile en France : situation et motivations des sages-femmes

Abstract : Objectives: The study is about the motivations of French midewives practising homebirth, knowing the unusual caracter of this practice in this country. It also discusses their current situation as well as the obstacles they have on a daily basis. Method: A descriptive and interpretative cross-sectional qualitative study was conducted. Eleven interviewed midewives were the subject of the study thanks to semi-structured interviews used to collect datas. Datas were processed through thematic content analysis. Findings and discussion: The midwives were practising home birth due to professional, personal or family transmissions. Global support benefits couples and midwives alike, and the physiology of the birth played an important role for both of them. Through the interviews, it emerged the notion that french midwives are facing multiple obstacles practicing homebirths. This could be due to the ignorance of their hospital colleagues as well as the lack of information about homebirth in the midwifery school. They noted the difficulty of interprofessional communication, the difficulty to be included to the local health care system, the lack of support from the hierarchy, or the professional insurance situation. These midwives face a certain loneliness in the exercise of a profession which seams not to be very financially valued. Conclusion: The motivations of midwives to practice homebirth are marked by a remarkable enthusiasm. The growing demand for physiology drives some couples to find many benefits such as taking back some control over the birth of their child. The institution affirms the importance of the freedom to choose the method of childbirth, but this practice continue to be crossed by multiple obstacles; for now, this practice is taking a distant place among other childbirth practices.
