Caractéristiques des parents refusant la vaccination en PMI et déterminants du refus vaccinal : étude réalisée auprès des médecins de PMI de Seine-Saint-Denis

Abstract : In France, mistrust of vaccines is frequently noticed, including in PMI services. This work aims to scale magnitude of vaccine refusal, identify main determinants of vaccination barriers and define characteristics of individuals refusing vaccination in PMI. We performed a cross-sectional study among parents of children between 0 to 6 consulting in PMI center for their child, thanks to a questionnaire filled by the doctor in case of vaccine refusal. On the study period, only 26 filled questionnaires have been brought back. The statistical analysis showed that individuals between 25 to 35 were the more refusing the vaccine for their child (73%). Infants aged from 6 to 24 months were the more often affected by the vaccine refusal (54%). The main concerns linked to vaccines were about risks of side effects, the doubt about the effectiveness of vaccination and the influence of pharmaceutical lobbying. The doctor in largely the main source of information about vaccination. The hepatitis B vaccine was the more rejected. The main reasons for vaccine refusal are the fear of adverse effects, the non-essential nature of vaccines and their excessive number. Vaccine refusal has become rare in the PMI population, probably in connection with the widening of the vaccination obligation since January 2018. Vaccination adherence can be enhanced by a better training of the general practitioner on this topic and by the emergence of reliable and easily accessible sources of information.
