Examen clinique de l'épicondylalgie latérale du coude en soins premiers. Une revue systématique de la littérature

Abstract : Musculoskeletal disorders (MSDs) are the leading occupational disease in France. The increase in the demand for care makes it difficult for patients to access primary care. Among MSDs, tendinopathies represent more than 30% of consultations in general medicine; the lateral side of the elbow is one of the most affected. However, exclusively tendon involvement of lateral elbow pain is called into question. The main players in this pathology are the general practitioner and the emergency physician, conditioning the diagnostic and therapeutic orientation; but their practice differs from that of musculoskeletal specialists. This results in excessive reliance on drug treatments and imaging. The objective is to determine the clinical signs allowing the diagnostic orientation towards a tendon, articular, ligament, nerve, or cervical origin of lateral elbow pain, by a systematic review of the literature carried out according to the PRISMA criteria. A search in 7 French-speaking and international databases between 1972 and 2021 made it possible to include 38 articles. The combination of Cozen's and Mill's tests and palpation of the lateral epicondyle allows the diagnosis of tendinopathy. The semiological analysis of pain can rule out a radial tunnel. An examination of the range of motion associated with screening and / or diagnostic tests explores a joint origin. 3 screening tests allow ligament orientation. And cervical radiculopathy research is needed to rule it out. Outpatient studies could determine whether this approach allows for better therapeutic orientation.
