Impact de la mise en place d’un protocole de nutrition parentérale au sein d’un service de réanimation néonatale

Abstract : Neonatal medicine is in constant evolution. Important progress has been made and nutrition has been considered as the core of the treatment strategy for premature babies. This study will show the established impact of a parenteral nutrition protocol on the period of time necessary to meet the nutritional targets and verify if safety as well as a neutral economic impact. It is a monocentric, observation-based, following a before/after protocol establishment model. 61 patients were under study, 30 of them being part of a control group (before the establishment of the protocol) and 31 in an experimental group (after the establishment of the protocol). This study showed that nutritional target was reached quickly most of the times before the establishment of the protocol. After its introduction, no improvements in term of reaching the nutritional target had been observed. Results can be explained by the profiles of the patients from the experimental group, very immature and in critical condition, but also by the partial establishment of the protocol. Incidence of infections on the central line has not been impacted by the introduction of the protocol and it level was very satisfactory. The cost did not change as well. The established protocol fulfills an ideal when it comes to nutrition in clinical practice. Doctors must prescribe a parental nutrition that should satisfy the nutritional needs of a premature baby, with a reliable bacterial safety and an inexpensive cost.
