Influence de la morphologie des surfaces osseuses sacrées et iliaques antérieures sur la survenue d’une arthrose sacro-iliaque : une étude cas-témoins

Abstract : Despite numerous shape variations, there are very few descriptions of morphologic predisposition to early sacroiliac joint (SIJ) osteoarthritis (OA). It is surprising, since it is the main axis for transfer of the weight of the upper body to the lower limbs. The anterior-middle part tends to show more degeneration signs and carries heavier weights. Therefore, our goal was to determine the influence of convexity and angle of these anterior bone surfaces on early OA. Design SIJ OA signs from 325 CT of patients aged between 54 and 56 years were analyzed in axial plan by two independent readers and categorized as “significant for the age” or not. Then, the anterior SIJ angle (ASIJA) was measured and convexity of anterior SIJ bone surfaces was visually characterized in sagittal plan as absent or “L-shape” (group A), marked or “C-shape” (group C) or intermediate (group B). Results Concordance between the two readers for OA classification was 73% (Cohen’s Kappa of 0.4). 47 SIJ were included in group 1 (significant OA) and 94 in group 2 (non significant OA). Mean ASIJA ±SD was 107° (±9.99°) in group 2 and 92.9° (±8.50°) in group 1. 16/17 group A SIJ were in group 1 when 33/77 group B SIJ and 0/47 group C SIJ were in group 1 (all p-values <0.001). Conclusion Sagittal plan seems useful to assess different SIJ anterior shapes. ASIJA and convexity of anterior sacral and iliac bone surfaces appears to be a risk factor of SIJ OA.
