Étude des freins et leviers à l’organisation d’une filière de prise en charge du tabagisme des patients reçus à la Ligue Contre le Cancer : étude qualitative auprès des professionnels de la Ligue en Gironde

Abstract : Introduction: systematization of smoking cessation support for smoking patients with cancer is an objective of the 2014-2019 Cancer Plan, then of the 2021-2030 ten-year cancer control strategy. It calls for a stronger involvement of all professionals in the patient care pathway, and an improvement in clinical practices. The main objective was to determine, from the point of view of professionals, the obstacles and levers for the organization of a treatment system for smoking in patients received at the League against cancer. Méthod: this is a qualitative study with thematic analysis of semi-structured interviews with 11 professionals from the League against cancer in Gironde. Results: the obstacles mentioned by professionals to their involvement in patients' smoking cessation are a lack of patients referred to the League, a lack of follow-up, a lack of time and training, a lack of discussion between professionals, the weight of the words of certain doctors going against smoking cessation, the absence of a prescriber of nicotine replacement therapy, and the brakes specific to patients. The levers already present are a favorable time and place, the presence of experienced and motivated professionals who already have the tools to assist their patients in smoking cessation, and the levers specific to patients. The levers to be developed are the establishment of training, the presence of a reference professional prescriber, a unity and exchanges between professionals in the care pathway. Conclusion: within the supportive care pathway offered by the League, the establishment of a smoking cessation process for patients could be beneficial. For this, it is necessary to remove the obstacles to the involvement of professionals in this organization, by developing training, tools, and interprofessional coordination centered on the patient. The LAST project appears as an opportunity.
