« Et si on parlait vasectomie ? » Étude des besoins et motivations pour une formation théorique des médecins généralistes : étude qualitative à visée exploratoire

Abstract : Introduction: while vasectomy is a widely practiced procedure in some countries, it remains marginal in France. Studies have revealed that general practitioners are interested in receiving information on permanent male sterilization. Thus, the research objective was to study the needs and motivations of general practitioners with regards to a theory-based training program on the practice of vasectomy. Design: this qualitative study using grounded theory analysis was conducted through a series of semi-structured individual interviews with general practitioners, carried out from December 2019 to December 2020. Results: most of the doctors interviewed (ten women and six men) had distant memories of vasectomy and had never used the information tools available on this subject. Our study confirmed that most physicians want to receive theoretical training on the practice of vasectomy. The expected benefits were remembering to bring up the option of vasectomy, reassuring patients, and presenting the arguments in favor of vasectomy. Nevertheless, some subjective factors hinder the development of communication about vasectomy in their practice (personal approach, forensic aspects, etc.) Two shared decision-making tools about contraception have aroused the interest of physicians, who envisioned training in the form of a presentation by a specialist (conducted face-to-face or remotely). Conclusion: general practitioners suggest broadening the discussion around vasectomy to many situations and speaking directly to men or moving toward couples’ contraception consultations. Training courses on this topic should be short and exhaustive and provide simple shared decision-making tools. The training should also enable physicians to work on the way in which they present the option of vasectomy and help participants to answer the question of « when and how to speak about contraception by vasectomy in general medicine consultation ? »
