Dispositif d’annonce culturel en oncologie thoracique intégrative en Polynésie

Abstract : The A Haere I Mua notebook is a cultural hopeful communication tool for patients (PT) with lung cancer (LC). The object of this study is to determine its optimal place within the care path of the PT. 85 PT with LC were included in a study on the diagnosis announcement, randomized into 2 groups, receiving the notebook during the initial announcement consultation (IAC), or remotely. A questionnaire analysis of the delevering bad news’s components, including the culture, was realized with PT and 29 health professionals (HP), doctors and nurses, over 1 year. 15 HP carried out anthropological interviews. The overall assessment of the IAC (OAC) of the PT, doctors and nurses seems to attest to its relevance (7.45 ; 7.16 ; 8.01; mean 7.54/10). The diagnosis of cancer is delivered (100%), the treatments are specified (chemotherapy 96%). However the PT have difficulty perceiving the invitation to dialogue of the HP (p=0.001), who hardly discern their emotions (p=0). Faced with the silence of the polynesian PT, the human dialogue posture of the doctor is more difficult to maintain than the technical one of the informant. Despite this, confidence is setting in (96%) and the PT engage in the care path (95%). The notebook does not improve the OAC (p>0.3). The doctor concerned with the announcement's success is inclined to use it during the ICA. The nurse attentive to the PT’s shock prefers to postpone it, even if it allows to talk more about culture (p=0.001). Facilitating the exchange about traditional medicine, in the respect for the PT’s cultural identity, it strengthens mutual understanding and trust. The notebook helps to unite the care team around the integrative principle of "working together" for the PT. It calls for training to the announcement with a cultural dimension. Optimized within the personalized care program, it can contribute to the emergence of a network involving all health care providers into the project of the Polynesian Cancer Institute.
