Prise en charge bucco-dentaire de l’enfant sous anesthésie générale : validation d’un score objectif d’aide à l’indication

Résumé

Despite the presence of guidelines, comprehensive care under general anesthesira appears to be most often guided by the practitioner’s personal choice. The objective of this work is to confirm a score to help in the indication of general anesthesia in pediatric dentistry. This score was applied retrospectively to children treated in 2018 and 2019 in the Oral Medicine Department of the Louis-Mourier Hospital (Colombes 92 – APHP). The analysis was conducted among 239 children, including 114 children treated without or with conscious sedation and 125 children treated under general anesthesia. The mean score obtained in children treated under general anaesthesia (m=12.59) was higher than that obtained in children treated with or without conscious sedation (m=5.91) (p=10-7). Scores related to age (2.20 ± 1.48 points / 1.24 ± 1.31 points p=10-7), need for care (4.53 ± 1.21 points / 3.32 ± 1.85 points; p=10-8), behaviour (4.08 ± 1.80 points / 1.03 ± 1.62 points; p=10-32), medical history (m=0.73 ± 1.35 points / m=0.07 ± 0.54 points; p=10-6) and additional factors (1.06 ± 1.57 points / 0.26 ± 0.44 points; p=10-7) are always higher in children treated under general anaesthesia than in children treated with or without conscious sedation.
This study confirms the validity of the present score to help the dentist taking care of children with severe carious disease. It appears less relevant to help the practitioner in complex pediatric surgery.
Malgré la présence de recommandations, le recours à l’anesthésie générale apparaît le plus souvent comme guidé par un choix personnel du praticien. L’objectif de ce travail est de valider un score objectif d’aide à l’indication de l’anesthésie générale en odontologie pédiatrique. Ce score a été appliqué de manière rétrospective aux enfants soignés en 2018 et 2019 au sein du service de Médecine bucco-dentaire de l’hôpital Louis-Mourier (Colombes 92 – APHP). L’analyse a été conduite auprès des 239 enfants, dont 114 enfants soignés au fauteuil, et 125 enfants soignés sous anesthésie générale. Le score moyen obtenu chez les enfants soignés sous anesthésie générale (m=12,59) est supérieur à celui obtenu chez les enfants soignés au fauteuil (m=5,91) (p=10-7). Les scores liés à l’âge (2,20 ± 1,48 points / 1,24 ± 1,31 points p=10-7), au besoin de soin (4,53 ± 1,21 points / 3,32 ± 1,85 points ; p=10-8) au comportement (4,08 ± 1,80 points / 1,03 ± 1,62 points ; p=10-32), aux antécédents médicaux (m=0,73 ± 1,35 points / m=0,07 ± 0,54 point ; p=10-6) et aux facteurs additionnels (1,06 ± 1,57 points / 0,26 ± 0,44 point ; p=10-7) sont toujours plus importants chez les enfants soignés sous anesthésie générale par rapport au score obtenu par les enfants soignés au fauteuil.
Cette étude confirme la validité du score pour guider le praticien face à une maladie carieuse sévère chez l’enfant. Il apparaît moins pertinent pour le guider face à des chirurgies complexes.
