Habitudes et difficultés de rédaction du courrier médical des médecins généralistes pour adresser un patient âgé aux urgences et expérimentation d’une fiche de liaison gériatrique d’urgence

Abstract : Introduction: the medical letter written by general practitioners to refer an elderly patient to the emergency department is often deficient. It needs to be standardised. Objectives: to explore the habits and difficulties of general practitioners in writing medical letter for emergency referral of patients aged 75 and over who do not live in a nursing home. Have these doctors try out a standardised letter called geriatric emergency liaison form, ideally to pre-fill, whose content had been validated by a group of experts, in order to judge its acceptability and improve it. Material and method: qualitative study by conducting individual semi-structured interviews with general practitioners based on the Basque Coast, with a maximum variation sampling. Doctors were individually interviewed again if they agreed to try out the form. The interviews were recorded, fully retranscribed and analysed continuously using the thematic analysis method. Results: fifteen doctors participated and reported several difficulties in writing their medical letter, mainly when they were on home visit. Fourteen doctors agreed to try out the form. Seven doctors were not able to try it out during the limited period allocated for various reasons, but five of them still wanted to test it in the future. The other seven doctors tried it out in different ways, either typed or handwritten, and they all wanted to use it in the future for their elderly patients seen in home visit. The main advantage was to limit the risk of missing certain information to be transmitted, especially on geriatric, social and ethical aspects. Conclusion: the form satisfies general practitioners and is an efficient tool for transmitting the information necessary for the emergency management of an elderly patient.
