Comment améliorer l’évaluation des effets indésirables sexuels ?

Abstract : INTRODUCTION – Neuroleptics are psychotropic drugs that are frequently prescribed. They have numerous adverse effects - including sexual ones – that impact patients’ quality of life and compliance to treatment. Though these effects may have significant repercussions on patients’ lives, it seems that they remain underestimated by psychiatrists and rarely reported by patients. The aim of this study is to better understand what enables or hinders patients under neuroleptic treatment and psychiatrists to broach the topic of sexual adverse effects. METHOD – We performed a qualitative study including 9 psychiatrists and 11 patients. A single questionnaire was used during a semi-structured interview to cross-reference the psychiatrists’ and the patients’ points of view. RESULTS – The participants talked about the role of sexuality and the difficulty to raise this topic in session. Women might speak less than men about the sexual adverse effects affecting them. They might also be less often questioned on this topic, as might older people. The assessment may be made easier when psychiatrists adopt a tactful and open approach. CONCLUSION – It seems necessary that psychiatrists should inquire on possible sexual adverse effects during patient follow-up. Our study found that being a woman, being elderly or single seem to be risk factors for under-assessment. The creation of specific training programmes for patients and psychiatrists, the possible appeal to a third party and the availability of a written support such as a questionnaire could improve the management of sexual adverse effects.
