Chirurgie bariatrique en trois temps : anneau, sleeve puis bypass, résultats à moyen terme d’une cohorte rétrospective

Abstract : Purpose: Bariatric surgery is the best treatment for obesity, the three main obesity surgeries performed worldwide are sleeve gastrectomy (SG), Roux-en-Y gastric bypass (RYGB) and gastric banding (LAGB). All bariatric procedures include risk of failing. The causes of conversion are insufficient weight loss, uncontrolled comorbidities, presence of side effects. Sometimes, the second procedure proves to be a failure as well. Three-stage strategies have been little described. The objective of this study is to measure the excess weight loss (%EWL) of three consecutive bariatrics surgeries according to the scheme: LAGB, SG and RYGB. Materials and Methods: In a retrospective cohort were included all patients who underwent a triple bariatric procedure on the LAGB, SG and RYGB scheme. Results: Eleven patients were included, average BMI of 48.9 ± 7.7. At the first conversion, EWL was 15.3% ± 23.2 after 6.8 years ± 3.3 on average. At the second conversion the two procedures EWL was 31.6 % ± 12.7 and all patients had positive weight loss, but none exceeded 50% EWL for an average of a total time of follow up of 9.8 years ± 1.3. At the end of the treatment, the average BMI was 29.7 ± 3.6 for 78.8% result of EWL ± 15.2 on average, all patients had at least 50% EWL. At the last known weight, the mean follow-up was 16.8 years ± 4.0. Conclusion: The possibility of a RYGB procedure should not be excluded even if the first two surgeries were well conducted and still were a failure.
