Évolution des lésions cérébrales globales et cérébelleuses à l’IRM en tenseur de diffusion dans le suivi à long terme après traumatisme crânien grave

Abstract : After a severe traumatic brain injury (TBI), tertiary post-TBI lesions may appear. These are characterized by cerebral atrophy and white matter (WM) alteration in the diffusion tensor magnetic resonance imaging (DTI). To date few studies have considered cerebellum indirect injuries during TBI. The purpose of this work was to study the long term evolution of tertiary post-TBI lesions in the supra tentorial level and in the cerebellum by means of TDI analysis. This retrospective work was carried out on a 17 patient cohort, on which 3 DTI were performed in a 10-year period post TBI, in comparison to a group control. Four periods were initially singled out: before TBI (control group), during intensive care, from 1 to 5 years and from 5 to 10 years post TBI.
Supra tentorial lesions showed a biphasic evolution with an alteration characterized by cerebral atrophy and disruption of WM integrity (higher MD and lower FA). This seemed to occur in the early stage with an aggravation in the first 5 years (p<0,05) following TBI and with a stabilization afterwards. Cerebellar lesions, more precisely those on the anterior lobe and on the middle cerebellar peduncle, seemed to follow a different pattern, with an alteration starting also from intensive care period (p<0,05), but without further aggravation within 10 years. Overall, the evolution of post-TBI cerebellar lesions appears different from the supra tentorial ones, with a slower rate of progression or even a stabilization. Considering these results, it would be interesting to explore the clinical outcomes of these radiologic anomalies.
