Évaluation standardisée des troubles psychiatriques dans une cohorte de patients consultant en psychiatrie de l’enfant et de l’adolescent en région PACA pendant et après le confinement lié à la pandémie de COVID-19

Abstract : Objectives: the coronavirus pandemic led to confinement measures in France. Several studies have shown that epidemics and associated health measures (quarantine, confinement) cause an increase in acute stress disorders, post-traumatic stress disorders, anxiety and depression in the adult population. In the pediatric population, few studies have been conducted on this subject. The primary objective of this study was to evaluate psychiatric disorders (DSM-5) in a pediatric population consulting with child and adolescent psychiatry in the Provence Alpes Côte d'Azur (PACA) region during and after the confinement related to the Corona virus disease 2019 (COVID-19) pandemic. The secondary objective was to evaluate somatization symptoms in this population. Methods: this multicentric prospective observational study included its participants during the confinement between March the 17th and May the 11th, 2020. At least one month after inclusion, we conducted age-appropriate semi-directed psychiatric interviews Kiddie schedule for affective disorders and schizophrenia present and lifetime version and Diagnostic infant and preschool assessment (K-SADS-PL and DIPA) as well as the Patient health questionnaire-13 (PHQ-13) which collects somatic symptoms. The protocol was approved by the Ethics cCommittee of the Côte d'Azur University (CERNI). Results: the semi-directed psychiatric interviews (n = 25) identified anxiety disorders (76.0%), post-traumatic stress disorder (36.0%), and depressive disorders (28.0%). The PHQ-13 detects a feeling of tiredness (72.0%), sleep disorders (56.0%), headaches (56.0%) and stomach pain (56.0%). Conclusions: our study observes the significant presence of both anxiety-depressive disorders and post-traumatic stress disorder (DSM-5). In addition, our study population developed symptoms of anxiety somatization during or after the confinement. The large number of lost to follow-up and the small number of participants limit the generalizability of our results. Standardized interviews allowed us to characterize our population; their use with a larger population constitutes a challenge for clinical research.
