Plans de prise médicamenteux réalisés en psychiatrie : vers un accompagnement pharmaceutique personnalisé à la sortie du patient

Abstract : In psychiatry, 75% of patients stop their treatment 18 months after initiation. Discontinuation of treatment is very feared by psychiatrists because it increases risk of relapse. It is essential that patients are involved in their care to allow good therapeutic adherence. Especially in psychiatry with a young population, chronic pathologies and a lot of treatments with significant undesirable effects. So we made medication’s plans adapted to the patient's cognitive level. Indeed, psychiatric pathologies and their psychotropic treatments often lead to cognitive disorders limiting the patient in his social life. In collaboration with psychiatrists, we have drawn up a reference system with two levels of language (simple, sustained) containing required information for therapeutic adherence to drugs. Before leaving hospital, we conducted interviews with the patients. Medication’s plans were given to help them in their daily life. An assessment of treatment comprehension was performed after the interview. Then, therapeutic adherence is monitored one month after the end of hospitalization. This study shows the value of a pharmaceutical intervention with a language adapted to the discharge of the patient in a population where therapeutic follow-up is difficult. It would deserve to be extended to a larger sample. The implementation of medication’s plans could develop even more at the Charles Perrens Hospital Center.
