Abstract : Introduction: exertional heat stroke (EHS) is an encephalopathy associated with hyperthermia occurring during or following intense and prolonged physical exercise, which can turn into something worse, a systemic inflammatory response syndrome and followed by a multi-visceral failure, likely to lead to death. Knowledge of biological data in heat stroke is incomplete in the literature, so our study seeks to describe them, leading to a proposal for practical application for unit physicians. It is also looking for biological predictive markers of severe form. Materials and methods: we collected biological data from eighty-seven patients suffering from exertional heat stroke, seventy-eight non-severe forms and nine severe forms (defined by a period of time in Intensive Care Unit, hemodynamic failure, organ failure associated – or not – with persistent neurological disorder), from the day of exertional heat stroke until the fifth week. Following biological parameters, hemoglobin, platelets, creatinine, AST, ALT, total bilirubin, CPK, and PT, were analysed using mixed effect linear means and regressions. Results: we observed the main disturbances in the first week with full recovery between second and third week. In non-serious EHSs, some parameters fluctuated within biological norms (hemoglobin, platelets, total bilirubin), or were poorly perturbated, such as creatinine (131 micromoles/L at Day 0), PT (74 % at Day 1), and transaminases (15 N at Day 2). In serious EHSs, we found a decrease of PT (35 % at Day 1) and platelets (118 000/mm3 at Day 1) and an increase of creatinine (152 micromoles/L at Day 0), of CPK (70 N at Day 1), and transaminases (70 N at Day 2). We were able to show that the difference in biological disturbances between severe and non-serious forms is statistically significant during the first week. Conclusion: our work has made possible to better characterize the biological data of exertional heat stroke as well as to identify specific biological parameters of severe forms. It also made it possible to feed the reflection on two different biological tables for a same clinical entity, exertional heat stroke: severe forms and non-serious forms which seem to correspond to the biological physiological fluctuations of the healthy athlete, usually observed in sports medicine.
