Association entre le score d’Aletti et les complications post-opératoires : proposition d’une prise en charge post-opératoire adaptée au risque pour les patientes opérées d’un cancer épithélial de l’ovaire

Abstract : Objective: Surgery for ovarian cancer is complex and aggressive. A surgical complexity score (SCS) or Aletti score has been described to predict the rate of postoperative complications. This work aims to verify the interest of the Aletti score in routine in a large cohort of patients with advanced epithelial ovarian cancer. Methods: All patients with advanced epithelial ovarian cancer who had undergone primary or interval surgery at Centre Oscar Lambret, Lille, France between 2006 and 2017 were included. Patient, disease and surgical characteristics were collected as well as 30 day postoperative complications. Univariate and multivariate analyses were performed to establish the association between the Aletti score and postoperative complications. Results: Two hundred and eighty-eight patients were included. Among them 73.3% presented high grade serous. A high SCS (≥8) was found in 40.3% of patients. The complication rate was 27.8%, mostly Clavien Dindo grade 3. SCS was significantly associated with the risk of postoperative complications in univariate (OR=2.87 ,p<0.0001) and multivariate analysis (OR=2.10 ,p<0.02). Perspectives : We propose a management adapted to the risk of complications in order to reduce the induced morbidity for low-risk patients and to detect complications earlier for high-risk patients. Conclusion: SCS was associated with the occurrence of postoperative complications in our cohort. This result supports its use in current practice to propose an individualized management in ovarian cancer surgery.
